Position:
LTC LPN for a Nursing and Rehab Facility

Shift:
  • 5 shifts per week including every other weekend
Requirements:
  • BLS
  • Current and Valid NYS LPN License
  • 1 year of geriatric or Med/Surg experience
  • One year of LTC or SNF experience
  • Charting system experience - Epic or Matrix a plus

Job description / details:
  • Top 10 Procedures: IV fluids, IV meds, foley catheters, gastrostomy tubes, Wound VACs, oxygen management, BIPAP/CPAP, fingerstick blood sugars, phlebotomy
  • Charting system Matrix Care
  • Follow instructions from RN and clinical leadership
  • Administer medication as prescribed by the physician
  • Giving injections of medications
  • Taking vital signs such as blood pressure, temperature, and weight
  • Basic wound care including cleaning and bandaging injured areas
  • Providing immunizations
  • Respond to patient and family questions over the phone
  • Able to assemble equipment such as catheters and oxygen supplies
  • Recording the patient's history

Must be skilled in balancing care for patients and completing practical daily administrative tasks.
