SUMMARY The Licensed Practical Nurse in collaboration with physicians nursing staff and other members of the healthcare team oversees coordinates and provides focused episodic care to Rutgers Health patients This care encompasses health promotion health maintenance or management of healthrelated problems that is delivered in facetoface or telephone encounters

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following Other duties may be assigned List Essential Duty Responsibility Statements
1 Explains procedures and treatments to patient to gain cooperation understanding and allay apprehension
2 Be able to assist with patients who are medically complex andor have sensitive medical or social issues
3 Reviews plan of care medstests with physicians as needed
4 Contacts pharmacy for prescription renewals for patients
5 Sets updownloads 24hour ambulatory blood pressure monitors
6 Documents all patient correspondence and correspondence with other pertinent health care providers
7 Administers prescribed medications teaches patients how to administer injectable medication within the New Jersey Board of Nursing scope of practice
8 Assist in the treatment and examination of patient
9 Maintains awareness of comfort and safety needs of patient
10 Observes patient records significant conditions and reactions and notifies supervisor or Physician of patient39s condition and reactions to drugs treatment and significant incidents
11 Responds to life saving situations based upon nursing standards policies procedures and protocol
12 Documents all nursing history and assessment for patients in their charts
13 Answers all clinical phone calls obtains history and with the physician developsadministers therapeutic plan
14 Fills out school forms DCPampP forms Social Security Forms writes letters reviews orders from home carenursing agencies with physician
15 Helps obtain precertificationauthorization with insurance company prescribed medications or procedures
16 Maintains proficiency in electronic medical records for documentation
17 Schedules procedures and Radiology exams
18 Participates in department or unit quality improvement activities
19 Coordinatesparticipates in research studies
20 Understands and adheres to Rutgers compliance standards as they appear in Rutgers Corporate Compliance Policy Code of Conduct and Conflict of Interest Policy
21 Performs other related duties as assigned

JOB REQUIREMENTS To perform this job successfully an individual must be able to perform each essential duty satisfactorily The requirements listed must be representative of the knowledge skills minimum education training licensure experience andor ability required Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions
Must be a graduate of an accredited school of nursing with a license to practice as an LPN in the state of New Jersey Must have at least 2 years of experience The ability to function in a team environment is required Basic word processing and computer skills are required
Credential Required Valid Registration as a Licensed Practice Nurse in the State of New Jersey
