Job Summary: F/T LCSW bi-lingual (English/Spanish) VIRTUAL/Telehealth
  • Video or Telephonic Sessions with patients providing psychological evaluation and Talk Therapy in Spanish from your remote, secure location.
  • Comfortable treating Adult and Geriatric Ages Patients in an outpatient setting. Patients will be in their homes.
  • Comfortable treating diagnoses including but not limited to the anxiety, depression, behavior disorders and trauma.
 
How our program works
  1. PCP will refer patients to Forefront Telecare provider to see.
  2. Care Coordinator will reach out to you confirming your availability and ability to take on a new patient.
  3. PCP will email/fax patient information through our secure connection to our Forefront team which will then be uploaded into our EMR.
  4. Forefront will coordinate with provider and patient to schedule initial evaluation/assessment via zoom or telephone.
  5. Provider will document in Forefront EMR
  6. Provider will have ability to schedule follow ups using Forefront Telehealth Now system to schedule their own follow ups based on their own availability.
Patients will be in their homes and will be checked for proper internet and technology requirements for either a video session or telephonic sessions prior to scheduled appointments and must be able to either login themselves for zoom or have someone able to do it for them or have ability to call in to the session using their telephone.  
 
Support
We support our providers by assigning you a care coordinator so you will be able to communicate with us promptly throughout the week regarding questions we may have regarding your schedule or other patient needs/requests.
We let you focus on patient care as we manage all the billing and collections side.

*Monthly Bonus based on how many patients you see each month.
